This course is open to any HCA or allied health workers currently involved in the data collection for patients with Long Term Conditions (LTCs). There will be a brief overview of the main disease registers – Diabetes, Hypertension and Respiratory conditions. Exploring disease management and offering insight to living with a LTC there will be discussion relating to chronic disease and the impact that this has on the physical and mental well-being of the patient throughout their life.

Making every contact count (MECC) will offer opportunities for the clinician to provide Health Promotion and utilising the cycle of change, providing the necessary tools to support patients when they attend for annual reviews. Not just looking at data collection – but considering the patient within this.
