This course has been designed for those HCAs supporting Long Term Condition reviews in relation to cardiovascular disease and hypertension. The day will offer an insight into vascular disease and the management of related conditions including hypertension.
There will be focus on health promotion and change management to support the patient in self-care and the HCA will be competent to manage the consultation with a greater understanding of the relevance of data collection.
